Add 1password vault support and customizable reasons

This merge request does three things:

  • It adds optional support for looking up passwords from the 1password CLI if installed
  • It moves the HotCRP -> IP Paris reasons into the options/config
  • It shows a warning if an unexpected HotCRP decision type is encountered (instead of defaulting to a Bad Grades refusal).
